What to bring

• Photo ID
• Insurance card (if applicable)
• Accident or incident report (for PI / WC / auto cases)
• Attorney contact info (if you have one)
• Any prior imaging (X-ray, MRI, CT) on disc or app

Your first visit

1. Intake forms — about 10 minutes
2. Comprehensive exam with one of our providers
3. X-rays if clinically indicated
4. Personalized treatment plan and recommendations
5. First treatment, in most cases on the same visit
